Short-term Rehabilitation

We offer comprehensive sub-acute rehabilitation services to aid in the recovery process after surgery, illness, or injury. Our team of therapists and healthcare professionals work together to develop personalized rehabilitation plans, helping patients regain strength, mobility, and independence.

Our short-term rehabilitation program offer a range of physical, occupational, and speech therapy services, up to 7 days per week.

Our therapist help residents reach their goals using personalized treatment plans that focus on reducing pain, improving mobility, increasing independence, and reducing the risk of future injuries. Our exceptional team of therapists and healthcare professionals work collaboratively to develop a personalized rehabilitation program for these patients.

Following joint replacement, our skilled therapists will develop a plan to help restore mobility and range of motion. Our targeted therapies help ensure a systematic recovery process, maximize a patient’s independence, enhance the individual’s quality of life, and assist with community re-integration.

With compassion and expertise, the South Shore rehabilitation team helps residents reach their highest level of functioning and regain their independence. Our certified therapists offer specialized education, adaptive equipment & technology ensuring comprehensive support to accelerate the recovery process.

Cardiac Rehabilitation is designed for those recovering from cardiac conditions such as heart failure and heart attack — as well as from post-surgical and non-surgical procedures.

Our multidisciplinary team assists residents toward a successful recovery by offering:

  • Individualized and comprehensive physical, occupational and respiratory therapies
  • Nutritional expertise and a heart-healthy diet
  • Wellness education to help adapt to new lifestyle choices

Skilled Nursing Care

The nursing staff at South Shore is trained and has the expertise to administer a variety of advanced medical treatments.

Specialized services and treatments offered:

For those with COPD, pneumonia, asthma, or shortness of breath, South Shore offers care for advanced respiratory needs, including Tracheostomy Care, BiPap and CPap treatments.

A complex wound is one that goes beyond simple wound dressing treatments. They can be either acute or chronic, and result from a variety of medical and/or surgical conditions. A complicated wound that is not able to heal properly can lead to further health complications, requiring additional surgical intervention.

At South Shore, we provide a comprehensive wound treatment plan to maximize healing. Our wound care team includes a Certified Wound Care Physician, Certified Wound Treatment Nurse, Dietician, and Infection Control Nurse.

We treat complex wounds and surgical conditions with a team that includes social workers, a dietitian, and a physical therapist. We utilize our multi-disciplinary team by offering care for the following conditions:

  • Amputations
  • Infected and draining wounds
  • Pressure ulcers
  • Diabetic ulcers
  • Venous stasis ulcers and peripheral vascular disease
  • Arterial ulcers
  • Ischemic ulcers
  • Traumatic and Surgical wounds

Infusion therapy assists residents with a variety of illnesses and conditions. Infusion therapy is utilized to help treat infections, maximize pain management, dehydration, and nutritional support known as TPN (Total Parenteral Nutrition).

The nurses at South Shore work closely with our residents to help them understand their medication treatments and the importance of proper medication adherence. Medication management reduces hospitalizations and can improve a patient’s well-being.

Long Term Care

South Shore offers a wide range of services to meet our residents’ personal care needs. Our staff is available 24 hours a day, 7 days a week, to cater to the needs of our residents. We provide our long-term care residents with assistance on activities of daily living, medication management, nutritional needs, and restorative therapies fostering a supportive and comfortable living environment. Our residents are also engaged in a variety of activities, programs, and religious services to keep them active.

Hospice and Palliative Care

Focused on enhancing quality of life, the South Shore inter-disciplinary team offers compassionate support, relieving discomfort, and respecting resident’s values and preferences throughout their disease progression. Our clinical team attends to the residents’ medical needs to manage symptoms and pain, so the resident can spend their remaining time in comfort.

In House Hemodialysis

By offering in-house dialysis, residents at South Shore get the benefit of staying on-site, eliminating the commute time, waiting room time, and inconvenience. Residents also get the benefit of staying in a familiar, quiet, comfortable place, which is an important amenity for elderly residents, especially those who are recovering from surgery or injury.
